Although the main goal of clinical guidelines is to enhance patient outcomes, considering their economic implications is equally crucial. Economic evaluations offer a comprehensive assessment of the costs and benefits associated with guideline development and implementation.

This article presents a review and critique of Brazil's national Pay-For-Performance (P4P) program for primary health care, with a focus on its key design features and a comparison to England's Quality and Outcome Framework (QOF).
